Salary:
- PAYE: £29.24 per hour
- Umbrella: £39.32 per hour
What You’ll Do:
- Complete integrated and multi-disciplinary assessments that are person-centred and strength-based, focusing on “what matters” to individuals.
- Develop care and support plans that define personal outcomes and measure the effectiveness of interventions.
- Maintain and manage a caseload, empowering individuals, carers, and communities to promote sustainable wellbeing.
- Provide accessible information and advice through an immediate response service.
- Apply Fair Access to Care guidance in line with Monmouthshire County Council policies.
- Undertake risk assessments and develop proportionate risk management plans where required.
- Support people through hospital discharge processes, promoting independence within community settings.
- Uphold individuals’ rights under the Human Rights Act and Mental Capacity Act, ensuring least restrictive practice.
- Carry out specialist assessments including Mental Capacity Assessments and Continuing Health Care assessments.
- Prepare reports and contribute to Adult Protection meetings, case conferences, Court of Protection proceedings, and Quality Assurance groups.
- Work collaboratively with communities, providers, and partner organisations to develop community-based solutions.
- Maintain accurate financial records and complete financial assessments in line with local and national frameworks.
What We’re Looking For:
- A qualified Social Worker with relevant professional registration.
- Experience working within Adult Social Care, including safeguarding and multi-agency working.
- Strong assessment, report-writing, and communication skills.
- Sound knowledge of the Social Services and Well-being (Wales) Act 2014, Mental Capacity Act, and Human Rights legislation.
- Ability to work independently while contributing effectively to a wider team.
Essential Requirements:
- Eligibility to work in the UK.
- Enhanced DBS check.
- Signed Confidentiality Agreement.
- Minimum of 2 years’ references/work history.
