One of our key clients is currently seeking a Primary Care Advanced Nurse Practitioner to join our vibrant, patient focused and innovative practice. The surgery is located just outside Cardiff city centre, easily accessible with public transport and allocated parking. The practice consists of approximately 6500 patients with a supportive clinical team of 2 GP partners, 1 Advanced Nurse Practitioner, 1 cluster pharmacist, cluster physiotherapy team and a cluster mental health team as well as an exceptional administration team.

Job Description

To provide and maintain a high standard of nursing care to the practice population with focus in the delivery of evidence-based practice for patients presenting with long-term conditions and acute problems. This includes screening, assessment, treatment and education and taking an active role in supporting the practice to maximise achievement against local and national targets in respect of the GMS contract and Unified and Supplementary services.


Experience

Registered Nurse with up to date NMC Registration and must have worked in a Primary Care setting with knowledge of current policies, protocols and national targets. Clinical skill must include: 

• Chronic Disease Management such as Diabetes and Respiratory
• Smear taker
• Child and adult immunisations
• Women’s Health
• Wound Management
• Health Promotion
• Men’s Health
• Cardiovascular Disease Management
• Travel Health

 The practice will support further progression such as Minor Illness, Clinical Assessment and Independent Prescribing.
